Build Skills to Safely Navigate Community and Access Services

Less than 14, Age 14, Ages 15-16, Ages 17-18, Ages 19-21, Ages 22-24

Goal:

To have each student gain the skills to safely navigate their community and understand the resources and services available to them.

Before, during and after high school students should be presented with opportunities to manage their own care, navigate their communities, and understand what supports they will need to be successful after they leave high school.

  • Participates in social activities with supports or independently
  • Volunteers in their community at non-profit/community services
  • Identifies future living plans
  • Eligibility WIOA services explored
  • Registered for Selective Services

  • Understands transfer of rights and responsibilities under IDEA
  • Prepared to apply for HCPF waivers (multiple)
  • Makes and Keeps appointments
  • Understands guardianship options/alternatives
  • Registered to Vote and knows the rights of people with disabilities
  • Practices budgeting skills
